The ACGME just sent out the first of Five Modules on "Educating Physicians for the 21st Century".
Module 1 "Intro to Competency-Based Resident Education" is attached along with the Facilitator's Guide...It takes about 20 minutes to run through the presentation...this one seemed pretty basic...
The other Modules (Practical Implementation of the Competencies, Curriculum Planning, Assessment Systems, and Educational Quality Improvement) will be forthcoming...this is on their web site opening page --> www.acgme.org

The ACGME posted the second of their five modules in a new educational resource "Educating Physicians for the 21st Century" is now available: Practical Implementation of the Competencies (5/11/06) I've attached the PPT presentation and along with the Facilitator Manual...
They are from Barbara Joyce...the BOTTOM LINE of her presentation is that EACH COMPETENCY can be taught in about every clinical setting...

MY COMMENT: While this is somewhat helpful...it does NOT address the difficulty or any standards in utilizing the Competencies for MEASURABLE OUTCOMES...

The ACGME just posted a new Conference for a September weekend in Chicago that looks interesting...of note, Hubert Dreyfus who cmae up with the Dreyfus Learning model (Novice, Competent, Expert, etc) will present...It looks pretty good...limited to 200 participants...$265...

The second posting is a Conference that will be more ABMS driven with a more practical view of Improving Patient Care...there is not a brochure...but here is the Abstract Submission form...

ABMS-ACGME Conference on Assessing and Improving Patient Care November 2 & 3, 2006

The American Board of Medical Specialties (ABMS) and the Accreditation Council for Graduate Medical Education (ACGME) are co-sponsoring a national conference focusing on Assessing and Improving Patient Care. The conference will be held on November 2 & 3, 2006 at the Sofitel Chicago O’Hare Hotel.

The conference is open to medical educators, credentialers, or anyone interested in teaching, assessing and improving patient care in undergraduate, graduate, continuing medical education and practice environments.

The first night of the conference will feature a poster session for presenting original investigations or projects (ongoing or completed) related to assessing and improving patient care. We are especially interested in receiving abstracts which focus on:

Tools to assess the quality of patient care provided by residents and physicians in practice
Improving patient care
The chronic disease model and improving patient care
Practice redesign
Closing the quality gap
Using incentives (pay-for-performance/pay-for-participation)to improve patient care
All submissions must be received by the ABMS office no later than August 15, 2006. Submissions must be sent electronically using the attached form to kback@abms.org. Please follow the format requirements outlined on the attachment. No substitutions will be accepted. Authors will receive confirmation of their submission upon its receipt in the ABMS office. Questions regarding the abstracts should also be sent to the same address. Display specifications will be provided at the time of acceptance. The abstracts will be reproduced as part of the conference syllabus

You may have seen earlier DRAFTS of the proposed revisions to the ACGME COMMON REQUIREMENTS for all residencies...I've attached the "blue line version" and the "impact statement" just released...comments due November 5th...The AHME Exec Committee will review and may respond...at a cursory glance, most of the changes consolidate and reinforce the role of the DIO...Charlie Daschbach

I have attached the proposed changes in the ACGME Institutional Requirements and the "impact statement".
There are not 'big' changes, but some that will need to be addressed...
--a change in resident contract that includes a reference to "expectations for faculty responsibilities and supervision"...and a reference to "non-promotion"

--policies for residents with disabilities

--education on sleep deprivation to be included in within physician impairment education

--GMEC must ensure that sponsoring institution monitors vendor interactions with residents and GME Programs

--policy on how we would continue programs in the event of a disaster...

Just when you thought the ACGME had explained enough about the new Common Program Requirements¡Kthey have just posted this 72 page explanation/rationale¡K¡¨Program Director Guide to the Common Program Requirements¡¨

It does link each requirement „³ an explanation/rationale „³ PIF specific question „³ (if applicable) Resident Survey Questionnaire „³ Documentation

Using the same format, it also gives an initial understanding on how the ACGME will begin judging each of the COMPETENCIES

And more specificity on their expectations on EVALUATION¡K

This gives us new tools for GMEC PROGRAM OVERSIGHT, INTERNAL REVIEW and RRC SITE VISIT PREPARATION¡Kwe all need to distribute to our Program Directors and take to the GMEC...and we ALL need a careful reading of the ¡§GUIDE¡¨¡K

I might assume that all AHME members involved with GME receive the ACGME's e-Bulletin...if not sign up...

last week 's bulletin had the IMPORTANT NOTICE (below) on the updated information on each institution in your Part 1 IRD...if you have not downloaded this, it might be a good idea to take a look...

DIOs can find the electronic Attachment 1 by accessing their IRD. More specific information about how it has been designed and will be used will be included in a forthcoming issue of DIO News. Charlie,
I not only downloaded IRD Attachment 1A but am working on it as we speak. It is a Horror....
We are waiting for a date for our Institutional review and I am preparing all our documentation. Yesterday I went into IRD Part 1 to find our new Attachment 1A, and in my case, Attachment 1A lists every citation for each program since our last Institutional Review, 5 years ago, (whether the programs sent progress reports, and citations were rescinded by RRC's or not). What is up with this? Has anyone else started to complete this Attachment 1A? Help!

(after calling Linda directly to talk OFF LINE)

For those of us who have pending IRC visits, this is a double-edged sword...on the one hand, many of my citations in programs have been addressed and we had done a mock-up of many of the 'trended' citation RESPONSES...on the other hand at least I can see the official ACGME listing of citations AND HOW THEY LUMP THEM into the ACGME requirement categories in Attachment 1A. I'm glad I can see that... and will make some changes right away in our document.

One thought is to use the responses to the citations in an annual report to the GMEC and organized medical staff ?

We will have to TRACK citation responses within our institutions much more deliberately...with documantation...and that is a good thing.

Still, it is sobering to see all your citations together...we had our own list here (in two or three word categories) ...but the ACGME's looks more foreboding psychologically with the entire citation listed...and as a DIO, knowing that many (hopefully all!) of these have been addressed helps...

Whether you are up for Institutional Accreditation (like Linda and me) or not, download your information and start back-tracking your citation responses...

...and I had just thought of taking a few days off now that the new housestaff are settled in !!!

The rationale is that by grouping "institutional" related citations , a GMEC that is providing appropriate oversite will be able to address these citations at an institutional level before the next set of site visits..be they RRC or IRC.

I whole-heartedly AGREE Chris !
It makes good MANAGEMENT SENSE...
As I stated , this will be part of the ANNUAL Report to my GMEC and OMS...

I am also going to use an internal code for citations that we would consider CLOSED or STILL BEING ADDRESSED...or NOT YET ADDRESSED (gulp!)...but I believe this will become a USEFUL TOOL once we use it...(It particularly helped me with how the IRC saw some of these trended in different categories. Before Citations had the actual RRC Req. attached to them, I had to guess how the ACGME would "trend" them...

Now the question becomes..."Should we place a RESPONSE on the ADS System for each Citation?"

I'll send a note to Pat Surdyk for a response on this...we had not always done so here up to this point...
